There are a number of ways you may choose to work with us at THRIVE Health Clinic that are available to purchase or book through our website. The terms and conditions in this document apply to all of our services and products unless alternative terms are explicitly stated.

All of our products and services require 100% prepayment or agreement to a payment plan (direct debited from your bank account) before your receive your product or can attend your appointment. 


Initial Naturopathic Consultations are only offered as a package of 3 consultations in order for us to provide you with a comprehensive health review and a short, medium and long term treatment plan. Consultations are structured this way as at THRIVE Health Clinic as we are committed to setting you up for success in achieving your wellness goals and we aim to support you in reaching a state of optimal health. 


We offer:

  • One-on-one Naturopathic health consultations and consultation packages:

You will receive a New Patient Form to complete before your first appointment. In your 45 minute consultation a qualified Naturopath will ask you a number of questions to collect your case history and to get an understanding of your current health status, before tailoring a treatment plan to best support, restore, maintain and optimise your health and wellbeing. 

  • Mini remote consultations called ‘Ask A Naturopath’. A virtual 10-minute health food shop-style chat with a Naturopath for those seeking guidance and advice on only one health concern. 

  • Natural therapy appointments for Ear and Body Candling, Foot Detox treatments and Infrared Sauna sessions.

  • Downloadable electronic resources (i.e. e-books, pdf handouts and guides) to provide concise health and wellness information for your personal use and education. 

  • Online articles and posts about health and wellness information and recipes integrating our website and social media platforms. 

  • Online videos and programs that may contain videoed educational content on specific health topics. 

  • Products and supplements available for purchase online, such as herbal and nutritional medicines, and natural health and household products. 

  • Functional testing for purchase online, this includes a questionnaire to ensure the test if right for you and your health status, and a personalised consultation to interpret your test results. 

 

 

OUR QUALIFICATIONS & RESPONSIBILITY 

THRIVE Health Clinic is founded and owned by Ebony Jordan. Ebony is a qualified Naturopath with a Bachelor of Health Science (Naturopathy) and she is a full member of the NHAA Naturopaths and Herbalists Association of Australia (NHAA; member number 156072). THRIVE Health Clinic keep up-to-date with all necessary professional insurances including Public Liability and Professional Indemnity.  

In offering the services and products listed on this website, we understand we have a responsibility to keep our skills and expertise up-to-date by continuing professional development, taking due care and due diligence, working within our scope of practice, applying best practice, and keeping up with the latest research developments in our field.

Naturopaths often work collaboratively and cross-refer with various health professionals in your health team (such as general practitioners, physiotherapists, kinesiologists, psychologists, chiropractors and osteopaths). If we think you may benefit from seeking care from another practitioner for a particular issue which we may be outside of our scope, we may refer you to another practitioner or clinic. With your consent, we may also write a letter to your other health practitioners to share your health information so that we are all working together as a team to achieve optimal results for your health.

PAYMENT TERMS

THRIVE Health Clinic requires 100% prepayment or agreement to a payment plan (direct debited from your bank account) for all consultation bookings or purchases from our website. Additional products or supplements recommended during your appointment can be purchased in the clinic or from our online client order system.

DELIVERY & SHIPPING

DELIVERY

One-on-one consultations may be in person or online. In person consultations will take place at THRIVE Health Clinic, 8 Bradburys Lane, Horsham, Victoria, Australia, 3390. Online consultations will take place via Simple Clinic or Zoom. Please visit the Simple Clinic website to read their Simple Clinic- Terms & Conditions and Simple Clinic - Privacy Policy. Prior to your initial consultation, you will be sent a New Patient Intake Form or Questionnaire to complete and email back to me before your appointment. It is important that you do this, as the information you provide will determine whether you have booked the correct service for you, and it will allow us to spend more time treating you during your appointment. If we do not believe the service you have booked will be safe or appropriate for you, we will contact you and provide you with appropriate referrals and suggestions.  

In person consultations: Please arrive to the clinic at least 5 minutes prior to your appointment start time. THRIVE Health Clinic is located in the Coles Carpark, there is free 2-hour parking in Bradburys Lane Coles Carpark. The clinic is accessible by foot from Firebrace Street, Roberts Avenue, McLachlan Street and Darlot Street, and there is a bus service available from Roberts Avenue, and a taxi rank from McLachlan Street. 

 

Online consultations (Zoom): Prior to the consultation, you will receive a reminder email with a link to the Zoom meeting. Please download and install Zoom prior to our consultation time download Zoom here. 10 minutes before the appointment please click the ‘join a meeting’ link, and wait for your practitioner to welcome you into the meeting room. If we cannot connect within 10 minutes of your booked appointment time your practitioner will call you and continue your consultation as a phone call.

SHIPPING

Any prescribed products will be arranged to be delivered to you via a patient ordering system (Vital.ly, Metagenics, or Oborne Health Supplies). Please note: each of these providers use different couriers, and the estimated delivery times for goods may vary. from order to order. 


APPOINTMENT CANCELLATION POLICY

At THRIVE Health Clinic we give our clients a lot of time and care, we respect that at times unforeseeable events occur in life, but we also ask for you to respect that we spend precious time away from our families to provide our lovely clients with our time, energy and care. THRIVE Health Clinic kindly requests 24-hours notice if you are unable to attend a scheduled appointment. If you notify us prior to 24-hours we will be happy to reschedule or refund your appointment at no extra charge.


If you have an emergency or unforeseeable circumstances arises that inhibit you from attending your appointment, please let us know as we may choose to exercise our discretion on each individual case and may choose to reschedule or refund your appointment. 


A 75% cancellation fee will be charged for late cancellations (within the 24 hours of your appointment time) or if you fail to show. For payments made in full before your appointment, you will be refunded 25% of your total booking fee. 


 

REFUND POLICY & CONSUMER GUARANTEES 

If you book an appointment at THRIVE Health Clinic and attend your appointment or treatment in full, you cannot ask for a refund once the service it completed. You are given every opportunity to seek information about our services. We strongly recommend that if you have any questions about a service that we provide, that you contact us first before booking and attending your appointment to gain clarification either by phone or by emailing ebony@thrivehealthclinic.com.au.


If you have booked a consultation or treatment package and change you mind about working with us, you have 7 days (for programs, packages and courses that run for 3 weeks or more) from the first day of service to discuss with us a refund. You will be refunded your program fee minus the cost of the services you have already received plus 50%.


If you feel that there is any problem with any of the services or products we provide at THRIVE Health Clinic, please let me know as soon as possible. We would like to understand what has gone wrong if you are unhappy so that we can address your concerns and try to find a mutually acceptable solution.


As per the Australian Consumer Law, if there is a major problem, we are required to:

  • replace the goods or provide the services again; or

  • if we are unable to do so within a reasonable time, pay the cost of the relevant goods or services supplied to you.

 

RESPECTFUL COMMUNICATION

THRIVE Health Clinic reserves the right to delete any comments on our website or social media accounts which are rude, offensive or unacceptable. We ask that you act and comment respectfully, polite and mindfully.

DISPUTE RESOLUTION & JURISDICTION

Your use of our website, social media accounts and these Terms are governed by Victorian laws. As outlined earlier in these Terms, by using the THRIVE Health Clinic website, products or services, you agree to communicate with us if you have any concerns, issues or complaints.

i) If you have any concerns, issues or complaints arising out of your use of this website, my products or services, or these terms and conditions, you agree to communicate with me with the intention of making a genuine effort to seek a win/win solution, and trying to resolve the dispute through negotiation and discussion.

ii) You are encouraged to contact us at THRIVE Health Clinic via email ebony@thrivehealthclinic.com.au so that we can attempt to seek a solution.

 

iii) All information exchanged during this negotiation process or any subsequent dispute resolution process, shall be regarded as “without prejudice” communications and shall be treated as confidential by everyone involved and their representatives, unless otherwise required by law. 

iv) If we are unable to resolve a dispute by negotiation and discussion within 30 days, we agree to seek assistance in finding a resolution with an external party such as an independent accredited mediator.


v) We agree that neither of us will commence legal action until, in the opinion of an independent accredited mediator, the potential for negotiation and mediation have been exhausted. 

vi) Everyone involved in the dispute agrees that they will not publicly or privately disparage any other party or anyone associated with them, and will act in good faith to refrain from any conduct or communication which might reasonably be expected to interfere with any other party’s business or personal interests.

 

vii) Regardless of where you live in the world, this agreement is subject to the governing law of Victoria. 

 

viii) If we are unable to resolve a dispute by negotiation and discussion, you may access the complaints procedure offered by my professional association the Naturopaths and Herbalists Association of Australia NHAA-Complaints-Policy-.pdf.


TERMS SPECIFIC TO CONSULTATIONS


  1. Bookings: You can book a consultation with us by booking via our website and social media platforms, if you do not have access to these platforms please email us at admin@thrivehealthclinic.com.au, ebony@thrivehealhtclinic.com.au or SMS us at 04## ### ###. 

  2. Reminders: Once you have booked an appointment with us, you will receive SMS and email reminders at least 48 hours before your appointment. We provide these reminders as a courtesy. You are responsible for remembering scheduled appointments.

  3. Intake forms and documents: After booking your initial consultation, you will be sent an email which includes intake forms and (if required) other documentation we require you to complete prior to your first appointment. We require this information in order to understand your background, health history and provide our Products and Services to you. If you have any questions or concerns regarding the documentation, please contact us.

  4. Consultation fees: All consultations and services require 100% full payment or agreement to a payment plan (direct debited from your bank account) with our payment systems Simple Clinic, Stripe and/or Xero at the time of booking. This is so we can secure your appointment. By continuing with your booking, you agree to pay the fees, including any cancellation or failure to attend fees state in these terms and conditions.

  5. Cancellation or failure to attend: Please see our cancellation policy as set out in this document. 

  6. Refunds: Please see our refund policy as set out in this document.

  7. Your obligations: We require you to:

  • attend all scheduled appointments and be on time for your appointments;

  • if you need to cancel your appointment, do so in accordance with our cancellation policy, as set out above;

  • pay the required fees in accordance with these Terms;

  • be courteous and respectful to our practitioners, therapists, staff, and other clients at all times;

  • complete any intake documentation we require and provide accurate and complete information about yourself. In particular, ensure you advise us of all current medical conditions, any known allergies, food intolerances, any medications or supplements you are currently taking, if you are vegan or vegetarian (as some Products are derived from animal origins), and if you are pregnant, breastfeeding or trying to conceive.

  • provide personal information to us as required to enable us to provide the Services to you (which we will collect, use, store and disclose this information according to our Privacy Policy available on our Site);

  • update your personal and medical information as required;

  • deal with us in good faith and in compliance with all applicable laws, these Terms and our Documents.

If you fail to comply with your obligations or these Terms or the Documents, we may at our discretion, cease providing Services to you.
